Complete Personal Pharmacist Package

This package is designed to give you full access to my services and priority status for 1 month. We will begin with a complete initial medication review as described below. If you are hospitalized or suffer a fall, I will complete additional medication reviews for you. I will follow up with you each week for 30 minutes to answer any questions that you may have. Any e-mail questions will be prioritized and answered between 7:30 am and 9 pm. You will also have access to a secured messaging platform.

Complete Initial Medication Review

I will send you forms to complete regarding your current medications and health conditions. I will review your completed forms and outline some suggestions, followed by a 45 minute web meeting or phone call to discuss. All recommendations discussed will be summarized and provided to you. You will also be added to my monthly e-mail newsletter (voluntary of course).

Hourly Rate

Do you need more than 45 minutes to discuss your medications during our initial comprehensive medication review? Is there another service you would like me to provide? I am available to ensure that you are getting the personalized service required to optimize your health. My hourly rate will be billed in 15 minute increments as needed.

Over the Counter Medication Review

I will discuss and make recommendations as appropriate for over the counter medication options for you. A list of all current medications will need to be provided by you. We will discuss for up to 30 minutes either by phone or web meeting.

Readmission Review

If you are hospitalized after our initial comprehensive medication review, I will perform a review when you are discharged from the hospital. This will ensure there are no duplicate medications, transcription errors, or inappropriate medications. My recommendations will be discussed with you during a 30 minute phone call or web meeting. We will also discuss any new medications.

Fall Review

Falls are a major cause of reduced mobility and poor health outcomes in older adults. If you suffer a fall, I will do a complete review of your medications to ensure none of them could be causing or contributing to your falls. I will mail or e-mail you a summary of my recommendations.
